Raw Milk: What We Actually Know
Raw milk is a tribal flashpoint. The sober version: real pathogen risk, thin benefit evidence, who is most at risk, and the UK and US legal picture.
Raw milk is one of those topics where both tribes lose their minds. One side treats it as poison, the other as a sacred superfood. The sober version is less exciting: the pathogen risk is real and well documented, the benefit evidence is thin, and most of the confident claims in either direction do not survive contact with the data.
We will say this plainly up front, because it is a RED-flag topic and we take that seriously. TFC makes no health-benefit claim for raw milk. We do not stock it, we do not tell anyone to drink it, and nothing below is a recommendation to. This is an honest examination, not a nudge.
Why do people want raw milk in the first place?
Because the instinct behind it is reasonable, even if the conclusion overreaches. People are tired of ultra-processed everything, they distrust industrial food, and raw milk feels like a return to something whole and traditional. That is an understandable impulse and we are sympathetic to it.
There is also a taste and provenance argument. Milk straight from a known farm, unhomogenised, does taste different, and knowing your farmer is a real value. None of that is nonsense.
The trouble starts when the romance gets dressed up as medicine.
What do the claimed benefits actually rest on?
Less than the enthusiasts imply. The strongest thread of evidence is from studies of European farm children, where drinking farm milk was associated with lower rates of asthma and allergy [5]. That is a genuine, repeatedly observed pattern and worth taking seriously.
But read it carefully. It is observational, not a trial. Farm children are exposed to a whole world of microbes, animals and dust, and untangling the milk from the farm is very hard. The researchers behind that work were explicit that they were not recommending raw milk, precisely because the infection risk outweighs an uncertain benefit. They even suggested the protective component might be preserved by gentler processing rather than requiring raw milk at all.
The other common claims are weaker. The idea that raw milk fixes lactose intolerance is not well supported. And the notion that pasteurisation guts milk's nutrition is largely a myth: heating kills pathogens while leaving protein, fat, calcium and most vitamins essentially intact. The nutritional gap between raw and pasteurised is small.
So the benefit case is thin and heavily caveated. That matters when you weigh it against the other side of the ledger.
What does the risk data actually say?
This is where the argument stops being symmetrical. The risk is not hypothetical, and it is not small.
A CDC analysis of US dairy outbreaks found that unpasteurised milk and cheese caused outbreak-related illness at a rate around 840 times higher per serving than pasteurised products, despite being consumed by only a tiny slice of the population [1]. A separate review of two decades of US outbreaks found that where states loosened raw milk laws, outbreaks tended to rise [2].
The pathogens involved are not mild. E. coli O157, Salmonella, Campylobacter, Listeria. In a healthy adult these often mean a wretched few days. In the wrong person they mean haemolytic uraemic syndrome and kidney failure, miscarriage, or death.
Then there is the newer worry. Since 2024, highly pathogenic avian influenza H5N1 has spread through US dairy herds, and infectious virus has been found in high concentrations in raw milk from infected cows, with human cases among dairy workers [3]. The reassuring flip side is that standard pasteurisation reliably inactivates the virus: controlled work confirmed heating raw milk at 63 and 72 degrees Celsius knocked H5N1 down effectively [4]. In other words, the exact step raw milk skips is the step shown to neutralise the newest threat.
Who is most at risk?
Some people should be kept away from raw milk entirely, no matter how you weigh the general debate.
- Children, whose bodies handle these infections far worse than adults.
- Pregnant women, because Listeria in particular can cause miscarriage and stillbirth.
- Older adults, whose immune defences are weaker.
- Anyone immunocompromised, by illness or medication.
For these groups the calculation is not close. The downside is catastrophic and the upside is a benefit that barely holds up in healthy people.
What is the legal picture?
Different, and revealing, on each side of the Atlantic.
In the UK, raw drinking milk is legal in England, Wales and Northern Ireland but tightly controlled. It can only be sold direct to the consumer by registered farms, at the farm gate, by vending machine, or by direct online sale, and it must carry a warning that it is unpasteurised and may contain harmful bacteria [6]. It is banned outright in Scotland. Tighter labelling and controls have been proposed.
In the US, it is a state-by-state patchwork. Some states allow retail sale, some allow farm-gate or herd-share arrangements, and some ban it altogether. Selling raw milk across state lines for human consumption is prohibited under federal rule. The legal messiness itself tells you the authorities regard it as a meaningful risk.
Where TFC lands
Neither hysteria nor romanticism. Raw milk is not poison in the sense of one sip being disaster, and the people drinking it are not fools. But the honest weighing is lopsided: a thin, confounded benefit case against a real, documented, occasionally lethal risk, with a newer viral wildcard on top.
So we make no claim for it, we would never put it near a child, a pregnant woman or an immunocompromised person, and if you choose to drink it as a healthy adult, we would want you to do it with clear eyes about the risk rather than a headful of superfood marketing.

The bottom line
What we actually know: the risk from raw milk is real and measured, the benefits are thin and mostly indirect, the danger concentrates in the most vulnerable people, and the law treats it as a hazard for good reason. That is the whole honest picture. Anyone selling you certainty in either direction is selling you something.
