Vitamin D: Sunshine, Supplements, and the Blood Test That Settles It
Sun first, supplement when winter or your skin makes sun impossible, and test, don't guess. The honest read on vitamin D, dosing figures, and the blood test.
Vitamin D is best thought of as a hormone your skin makes from sunlight, not a pill you are meant to remember. Sun first is the honest position: it is how humans have always made it. But at high latitudes, in winter, and for darker skin, sunlight alone often is not enough, and that is exactly when a supplement earns its place. The one thing that settles the argument for you personally is a cheap blood test.
Where does vitamin D actually come from?
Your skin. When UVB light hits it, a cholesterol-derived compound in the skin converts to vitamin D, which your body then activates. It behaves less like a dietary vitamin and more like a hormone, which is why it touches so many systems.
That is the case for sun first. A short spell of midday sun on bare skin, in the right season, can generate more vitamin D than most tablets deliver, for free, in the form your body evolved to use. Food gives you some, oily fish, egg yolks, liver, but for most people sunlight is the main event.
Then why do so many people fall short?
Because the biology has hard limits, and modern life runs straight into them.
The big one is latitude and season. Above roughly 35 degrees of latitude, which includes all of the UK and northern Europe, the winter sun sits too low for meaningful UVB to reach the ground for months at a time. A classic 1988 study measured this directly: in Boston and Edmonton, winter sunlight simply did not produce vitamin D in skin, whatever the weather [1]. Britain effectively has a vitamin D winter from about October to March.
Then there is skin tone. Melanin is a natural sunscreen, so darker skin needs considerably longer in the sun to make the same amount of vitamin D as pale skin [2]. At northern latitudes, that is a real driver of deficiency.
Add the rest of modern life, indoor work, covered skin, sunscreen, older skin that synthesises less, and the just go outside advice quietly stops being enough for a large part of the population for a large part of the year.
When does a supplement make sense?
When sunlight cannot do the job, which for many people is a predictable, seasonal thing rather than a personal failing. The reasonable cases:
- Autumn and winter at northern latitudes, when skin synthesis all but stops
- Darker skin tones living far from the equator
- People who are mostly indoors, cover up, or are older
- Anyone whose blood test comes back low
This is where the sunlight-over-pills instinct meets reality. We would always rather you got sun when the season allows. We are not going to pretend a Manchester January offers that option.
How much did the trials use?
Here are the numbers as what the research and guidelines describe, not as an instruction for you.
Supplementation trials in adults commonly used somewhere between 1,000 and 4,000 international units of vitamin D3 a day [5]. The 2011 review of dietary reference intakes set 4,000 IU a day as the tolerable upper intake for adults, meaning the level regarded as safe for long-term daily use without monitoring, not a target to aim for [4]. A 2023 review of long-term supplementation at 3,200 to 4,000 IU a day found no consistent pattern of harm at those amounts [3].
More is not better here. The large VITAL trial gave healthy adults 2,000 IU a day for years and found it did not reduce heart disease or cancer in a population that was mostly already replete [6]. Vitamin D corrects a deficiency well. It is not a performance drug you can megadose for extra credit, and very high intakes carry their own risks.
Because this is a fat-soluble vitamin that accumulates, treat those figures as context, not a prescription. Read the label on any product, and if you are pregnant, on medication, or unsure, check the amount with your GP or pharmacist before committing to a daily habit.
The blood test that settles it
Everything above is general. The only way to know your own status is to measure it, and it is one of the genuinely worthwhile tests.
The marker is 25-hydroxyvitamin D, or 25(OH)D, a simple blood test your GP can run or you can buy as a finger-prick kit [5]. It replaces guesswork with an actual number. Test in late winter, when your level is at its lowest, and you learn whether you have a real shortfall, a comfortable level, or the rare case of too much.
Testing beats guessing in both directions. It stops people who are fine from taking pills they do not need, and it catches the genuinely deficient who assumed a bit of summer sun had them covered. The bottom line
Sun first, supplement when the season or your skin makes sun impossible, and test rather than guess. Vitamin D is a hormone you are built to make outdoors, the winter months at British latitudes genuinely get in the way, and a cheap 25(OH)D test turns a guessing game into a decision.
